Registration Requirements and Guidelines
By submitting this form, you certify that the information is accurate and you acknowledge the responsibility to keep this record correct by informing the Student Organizations Office of any changes in the information and/or in our constitution and by-laws. I also give permission for this information to be distributed by the Student Organizations Office on their website directory and office publications.
All Registration materials must be submitted by Monday, September 12th. Incomplete forms will not be accepted.
Please print and fill out the additional registration forms in order to ensure that your registration is complete. Completed forms must be turned into the Center for Student Involvement.
Please use the following checklist to confirm that your registration is complete. All items need to be completed by September 12, 2016 at 4:00 PM to be considered a Registered Student Organization.

- Officer Acknowledgement form
- Resource Center form
- Confirm that you have an updated Constitution filed
