Student Leadership Awards

 The Center for Student Involvement hosts an annual reception held in the honor of the recipients of eight Student Leadership Awards. The nomination forms for these awards are below. Please click on the graphic for the award you wish to nominate someone for.

Please remember that the nomination forms are due at 4:00 PM on April 4, 2018. 

Award Recipients will be invited to a Dinner Banquet held in their honor on April 25, 2018.

Preferably, submission of nomination forms should be sent via email to However, please submit the Organizational Portfolio in a physical copy. In the past, these have been submitted within a 3-ring binder. The design and presentation is up to each Student Organization.

Please Follow These Guidelines:

  • Follow all instructions regarding the nomination process posted on this webpage and on all nomination forms
  • Three copies for every recommendation letter must accompany each nomination form
  • Three copies for every Organizational Portfolio must accompany the nomination form for Outstanding New Student Organization of the Year Student Organization of the Year. **The guide for creating an Organizational Portfolio can be found below.
  • Each nominee must submit a Grade Authorization form (found on this webpage) that must be submitted to the Center for Student Involvement.
