How do I request access to University records?

Requests can be submitted as follows:

Is there a form I can use?

Yes, you can complete and electronically submit the Open Records Request Form or complete and print the Open Records Request Form and submit it as listed above. The form must be dated. If you submit an email request without using the form, your name must be included in the email.

You may also use the form provided by the Kentucky Office of Attorney General.

When will a response be provided?

A response will be provided within five business days. Please note the response time begins on the first business day following the date the request is received. If the records cannot be located or prepared for inspection within that time you will be notified about the cause of the delay and you will be provided the earliest date when the records will be available.

Is there a cost?

If you request a physical copy, or if the request can only be fulfilled by providing a physical copy, the cost is $.10 per page for black and white copies on 8½” x 11” paper plus cost of postage. Requests for records in a nonstandardized format may result in additional costs.
