Peace Corps Release Form

For data assessment purposes, Murray State University Peace Corps Prep program will release information on a regular basis to the Peace Corps. The Release of Information Form may be signed to give permission for this information to be released. The information that will be released will be limited to name, date of admission to the Peace Corps Prep program, date of graduation, and whether the program was successfully completed.
