2019 Wellness Pledge
Eligibility Criteria
Must be enrolled in Murray State’s health plan - AND -
Must elect Pledge during Open Enrollment 2019

Phase 1: $75 paid in January 2019
If enrolled in a HDHP plan, the $75 will be deposited in your HSA account.
If enrolled in a PPO plan, the $75 will be discounted from January health insurance
premium.
1. Complete a Real Age Test (formerly known as the Well-Being Assessment or WBA) OR the Wayne Corp General Health Risk Assessment: January 2, 2019 – February 22, 2019
Must be completed by February 22, 2019

2. Attend or complete two Racer Wellness sanctioned events between January 1, 2019 – May 31, 2019

level)
- Speaker - Dr. Amanda Joyce: Happiness in the Workplace.
- Bring your lunch to the Curris Center while taking the class.
- Sign-in sheets will be available for Wellness Pledge credit.
Paducah Campus Health Professions Fair: Monday, February 25, 2019 | 8:30am - 10:30am | Paducah Regional Campus:
- This event will include interactive activities and information booths as part of the University's wellness initiative.
- The Living Colon display will be set up for participants to walk through at this event.
- Sign-in sheets will be available at the Exercise Science booth for body composition and grip strength to obtain credit for the Pledge
Spring Health Mini Fair:
Spring "Mini" Health Fair: Wed. April 24 | 3pm - 5pm | Alexander Hall Atrium
Spring "Mini" Health Fair: Wed. May 1 | 7:30am - 9:30am | Curris Center 3rd level
- Sign-in sheets will be provided to show proof of participation.
- Each Fair will offer a total cholesterol screening (please fast for 12 hours prior), blood sugar, heel scan screening, blood pressure, BMI, waist circumference, and education.
Healthy Lifestyles Challenge:
More information coming soon! This challenge will take place online Healthy Lifestyles.
Proof of participation will be obtained from Healthy Lifestyles for Wellness Pledge
credit.
Racer Wellness group fitness class: Spring 2019 schedule:
- Attend a minimum of one Racer Wellness group fitness class (classes are located in Carr Health).
- Classes are offered several days/time per week.
- To receive Wellness Pledge credit for this, please email lsmee@murraystate.edu with the name and date of the class attended within 5 days of completing the class.
Employee Assistance Program (EAP) Soft Skills Course:
- Complete one of the many available soft skills courses and submit the certificate of completion that is generated at the end to msu.racerwellness@murraystate.edu for Wellness Pledge credit.
- Access the Wayne Corp website, log in, and click on "Soft Skills Courses".
- Wayne Corporation Website Instructions.
This is an evolving list and may be added to throughout this time period.

2. Obtain an annual physical or women’s examination between October 1, 2018 – August 31, 2019.
COMPLETE BY: September 1, 2019
- Physical must include lipid panel
- Women's examination must include Pap smear if medically appropriate
3. Compete an additional screening between October 1, 2018 – August 31, 2019.
COMPLETE BY: September 1, 2019
Eligible screenings:
- Bone density screening
- Colonoscopy
- Mammogram
- Prostate preventive examination
- Type 2 diabetes screening (HGB A1C)
- Dermatology skin screening
- Lipid panel (May be obtained at the fall and/or spring Health Fair)
- Dental examination
Must carry Murray State insurance in order to use the dental exam for the Wellness Pledge screening - Vision screening
Must carry vision insurance or use the health plan in order to use the vision exam for the Wellness Pledge screening
Notes
- If Phase 1 is not completed, employees will incur a monthly penalty of $22.50 from September 2019 – December 2019.
- If Phase 2 is not completed, employee will incur a monthly penalty of $60 from October 2019 – December 2019.
- Reasonable alternatives and accommodations will be made available in compliance with HIPAA & ADA.
